1. Insert card into the punch machine (cut corner top left)

2. Return carriage to Index 1

3. Press keys 1 and 5 of top keys 9 1

NOTE: Keys may be pressed sequentially or simultaneously

4. Press space key

5. Press keys 2 and 5 of top keys 8 2

6. Press space key

7. Press keys 1 and 4 of top keys 1 3

8. Press space key

9 Press keys 3 and 4 of top keys 3 4

10 Press space key

11. Press keys 1 and 4 of top keys 1 5

12. Press space key

13. Press keys 1 and 4 of top keys 1 6

14. Press space key
